For the Fall Forum 2023, the SNP Alliance is offering a new educational opportunity for meeting participants. On Sunday, October 22, 1:00PM -- 4:00PM, two intensives are being offered. Registration for these intensives is separate from full meeting registration with an additional registration fee. To ensure participants' additional resources are well invested, registration lists will be checked on site to ensure only registered participants participate in the intensive. Onsite registration for the intensives is not available. Two intensives are offered:

Intensive Option 2: The Art & Science of Engaging & Evaluating Solutions to Complex Social Risk Issues for Duals -- Experienced Plan Insights and Discussion
SNPs and MMPs have been working with partner organizations for many years to address social risk issues such as food insecurity, housing instability, transportation difficulties, and social isolation for their dual members. Areas of continued work are: engaging the individual, aligning efforts with providers/CBOs, and evaluating results through multi-source data analytics. As plans collaborate and partner with providers, CBOs, and communities, they learn what works and what doesn’t and modify their approaches. This intensive will provide a framework and foundation to understanding the issues, and feature industry and health plan experts sharing expertise. Through a combination of presentation, facilitated discussion, and Q&A, everyone attending will participate.

Learning Objectives:
  1. Understand current status around screening and addressing social risk issues for dual plans
  2. Learn barriers and strategies for engaging dual members (beneficiaries)
  3. Describe approaches for evaluating results and recommendations for using data analytics to assess impact
